On March 9, 2022, Horizon Actuarial Services, LLC announced that their servers were accessed without authorization between November 10, 2021 and November, 11, 2021. The data breach involved names, dates of birth, Social Security numbers, and health plan information. The data was obtained by the hackers and used to extort Horizon Actuarial for an undisclosed sum of money.
